It's an improved (and sexier) descendent of the Maxon/Ibanez branded PQ-9:

The 2 knobs left of the Mode switch are Hi boost/cut, Mid boost/cut; 2 knobs right of the switch are Mid Freq, Lo boost/cut.
With the switch down, all 3 bands function as shelves and the mid freq knob is disengaged.
With the switch up, the mid band is sweepable/parametric.
And the 2nd, rear toggle (usually limited to the stompbox version) switches the buffer in or out of the circuit.
I keep the mids parametric and use the buffer to make the most of the stock '81 DMM (along with the TB loop).
It's ideal to prevent loading on the DMM's 10Kohm input, especially with the 69/fuzz face in front of everything.
