Just picked this up from the PO.
It's a Si Fuzzrite w/ a pair of TZ82s and 0.05 uF flat ceramic caps.
1968 pot codes, but with the switch running through the board like the earlier Ge Fuzzrites.
Also has the short V/T knobs off the guitars like the early ones.
I'm guessing it's a transition fossil between Ge and the taped board Si Fuzzrites.

Just got this back from a nice repair by my mate JC, the electronics genius.
When I boughted it, it didn't work and looked like this:



Now, it works and sounds great. He replaced the crapped out pot, caps, and JFET. I got a new base and stomp pad from a Dunlop Fuzz Face, and looks like this:
