the juxtaposition of tiny sounds preceding crushing devastation is great. not exactly doom, but the Brainbombs album Obey does something like this. It's the darkness before the light. if mass murder and anal rape is considered the light.

Cello doom, I deliver. Saw these ladies open up for A Silver Mt Zion. It was rad. They were rad, and like every other rad Portland band they break up after ten minutes and three gigs. http://correspondences.bandcamp.com/

bagpipes. they're awesome and so haunting and powerful. perfect for intros, outros and interludes and such. i'd love to have a chance to incorporate some into a recording.

digging this. I wish i could afford all the stuff thats been mentioned so far. bagpipe cello timpani didgeridoo with guitars and normal drums with a hammond/leslie and some cannon ala 1812 overture..... yeah.

Also, check out Queen Elephantine for some drone-y/Indian tinged doom. Teeth of the Sea had a really weird/rad record come out a couple years ago called Orphaned By The Ocean that I remember having a bunch of really strange instrumentation (but the actual details escape me because it's on another harddrive and I was smoking a relatively large amount of pot at the time), dunno if they're still around, and Camel Of Doom also used Saxophone and some other more traditional instruments.
And Teeth of the Sea is apparently impossible to fucking find anymore, but one of the tracks is on the tube:
