Vulgar rules. "I'm gonna make hate to you" could be silly but is absolutely horrifying.
Antichrist and Cannibal Holocaust are at the top of my Fucked Up Movie list. Grave of the Fireflies too. Halloween III as well because as awful a movie that it is, the ending is pretty god damn unruly. All Your Children Were Eaten By Bugs, What.
Dagon is the only Lovecraft movie I've seen that captures his vibe, the others are either goofy or gross. So that qualifies as fucked up, to an extent.

I've only seen Antichrist and Dogville. Need see more now.
And now that I've read the entire thread...
Dogville might be the single most disturbing experience I've been privy to since reading The Satanic Verses. Both qualify as mindfuck, no gore required. Love both with all my essence.
Oldboy, Martyrs, and Jesus Camp. Yep, those all qualify. Oldboy still has the best fight scene I've witnessed. Lady Vegeance isn't nearly as good BUT is more disturbing. Jesus Camp made me almost as pissed off as The Corporation did. Had to drink to make it through that one.
Martyrs is a perfect balance of gore and psychological trauma. Excellent flick.
The Shining and Rosemary's Baby. Classic fuckedupedness.
Ichi the Killer and Battle Royale I both love, but don't consider either one disturbing. Great, but funny in a lot of ways. If I'm laughing, that takes the edge of disturbing off for me.
Inland Empire is either Lynch's worst or best film, I still can't decide. I like Eraserhead and Mulholland Drive the most, but love them all really.

Bug.....haven't met anyone else who's seen this movie. FUCKED UP! It was a Broadway play for years before it was a movie.
Ravenous is killer AND funny.
Grimm Love is pretty disturbing in a psychological kinda way.
Into The Wild will make you think twice about escaping civilization and being a loner.
Enter The Void is awesome.....great sex scenes as a bonus.
Sweeney Todd is the only musical I'd ever recommend.
The Strangers was disturbing and kind of sad.
I Spit On Your Grave (remake) was an awesome revenge flick.....especially if you've been raped or know anyone who has.
Katie Bird is definitely fucked up.
Sorry if any of these have already been posted.....these were just off the top of my head.

Bug was cool. i like that guy that looks like he's trying to channel Christopher Walken.
or....... does he even have a choice? maybe Walken uses his amazing Walken-Powers to force himself through the dooder?

Yeah....I do see some resemblance. Walken-Powers.....hmmmmm, kinda like that weird ass Malkovich portal movie that I forget the name of.
That same guy in Bug was in Before The Devil Knows You're Dead.....another messed up film. A Phillip Seymour-Hoffman classic.
I'd let Marisa Tomei just ol' fashion rape me any day.....

Bug was great, definitely a bit of a renaissance for Friedkin (who directed The Exorcist way back when).
I don't know that I'd call "Before The Devil Knows Your Dead" fucked up, but (on the director's kick) definitely a seriously awesome resume piece for Sidney Lumet(he's dead now, but just look at
his IMDB page--and proof that some directors, at least, can make great movies forever, instead of a brief period of time where inspiration meets talent.
The Malkovitch movie is called "Being John Malkovitch" btw, another great Kaufman screenplay*.
*If y'all haven't seen Adaptation, fix that immediately.

Definitely good......very realistic and interesting take on the experience of a foreign culture, philosophy, death/rebirth, drugs and of course SEX!!!
If you're interested in Buddhism or the Book of the Dead, I highly recommend it.

Cannibal Holocaust was fucked up merely for the gruesome animal scenes. I'm not a PETA person, but that was just fucked.
